Hello, I am Dr. Hong Myeong-hwi, Chief Physician at HyungInJae Korean Medicine Clinic Gangnam.
A face may look even at rest, yet one corner of the mouth can seem slower in a smiling photo. One neutral photo and one expressive photo cannot explain why. Camera angle and the size of the expression may differ, and a longstanding difference may simply have caught your attention that day.
In consultation, I start with when the change was first noticed, which expression shows it consistently, and how the movement feels. Onset matters. A newly weak or drooping face together with a speech change or weakness in an arm or leg is not a routine logging situation. Call emergency services or go to an emergency department immediately.

Separate the resting appearance from what happens during an expression
Use a relaxed, straight-ahead view as the baseline. Then note whether the difference appears with a natural smile, a broad smile showing the teeth, or raised eyebrows. Identify the area, such as the eye, cheek, or corner of the mouth, and whether the difference appears as the expression begins or while it is held. If useful, one video of about 10 seconds in consistent light and at a consistent distance can show the order of movement.
There is no need to force the expression repeatedly or score small differences with a ruler. Faces are not perfectly symmetrical objects, and a photo or video is not a diagnostic test. I use the record to see whether the resting and moving patterns differ consistently, not to enlarge an appearance difference into a conclusion.
Write down the first date and whether older photos show the same pattern
Record when and where you first noticed the difference. If available, compare one or two older photos showing a similar expression. Note whether it was present years ago, appeared over the last few days, remains the same through the day, or is most noticeable after fatigue or prolonged speaking. That is enough detail.
Do not invent a cause when older photos are unavailable. A difference that seems longstanding and a newly reduced movement can require different priorities. Even if a new weakness improves after a short time, stop logging and seek prompt assessment from an appropriate medical service.
Add sensation and day-to-day function to the movement note
Note pulling, numbness, tingling, or pain during the expression. Record functional changes separately, such as difficulty closing an eye, a dry-eye sensation, liquid escaping from one side of the mouth, or speech sounding different from usual. Include the dates of recent facial injury, dental work, surgery, infection symptoms, or a newly started medicine.
This is not a list of causes. Numbness or eye discomfort alone cannot identify a specific condition. I treat the log as a clinical-history aid that helps identify which examination or medical assessment may be needed; it does not replace diagnosis.
Sudden facial weakness with speech or limb symptoms needs emergency action
Call emergency services immediately if one side of the face suddenly droops or becomes weak, especially with weakness or numbness in an arm or leg, slurred or difficult speech, sudden vision or balance trouble, or a sudden severe headache without a known cause. Note the time the symptoms began. Do not drive yourself to the hospital.
Do not wait even if the symptoms disappear after a few minutes. New sudden facial weakness on its own, or difficulty closing an eye, also needs same-day assessment by an appropriate medical service. A facial-asymmetry consultation or Korean medicine care cannot replace emergency diagnosis, and no correction of appearance or movement can be promised in advance.
Frequently Asked Questions (FAQ)
Is it facial asymmetry if one corner of my mouth rises less only when I smile?
One photo or one expression cannot establish a diagnosis. Compare the resting and moving patterns and record onset, consistency, sensation, and any change in function.
How much expression video should I take before a consultation?
One video of about 10 seconds, taken at a consistent distance and in consistent light, is enough to show the expression where the difference appears. Do not force repeated movements or measure the face for a score.
My smile suddenly looked uneven but now seems normal. Can I wait?
Do not delay assessment for newly developed facial weakness, even if it resolves. If it occurs with speech difficulty, arm or leg weakness, or another neurological symptom, call emergency services or go to an emergency department immediately.
Can an expression log determine treatment by itself?
The log helps describe onset and movement differences. It cannot replace an examination or any necessary medical assessment; the cause and appropriate scope of care must be evaluated individually.
If a facial difference repeatedly appears only with a certain expression, briefly record when you first noticed it, the expression that shows it, how the movement feels, and any accompanying symptom. A single video of about 10 seconds under consistent conditions is enough to explain the movement sequence during consultation.
HyungInJae Korean Medicine Clinic Gangnam can review the record alongside your current condition during a consultation about expression-dependent facial asymmetry. Suitability and outcomes vary by individual. Sudden facial weakness, speech change, or arm or leg weakness takes priority over an appointment or symptom log; call emergency services or go to an emergency department.
※ Symptoms and the appropriate scope of care vary by individual. Consult an appropriate medical service for an accurate diagnosis.
